When this issue happens, can you tail the application server log and let me know what error is observed ? Or you can also do the following:
1.Stop Jboss.
2.Remove the cache by deleting the following directories:
jboss_home/standalone/tmp,
jboss_home/standalone/log,
jboss_home/standalone/data

3.Start Jboss
5.Reproduce the issue
6.Attach the newly generated jboss_home/standalone/log/server.log to the case notes.

I have import the records.zip in application and restart Application Server, but it still fails....

Dear Tejas Kargutkar ,

I have encountered the same problem; I create the new Appian directory by installation .exe in my 2 app server then I sync the <APPIAN_HOME>/_admin/accdocs1, <APPIAN_HOME>/_admin/accdocs2, and <APPIAN_HOME>/_admin/accdocs3 with a shared folder by setting the conf.suite.BASE_PATH=<SHARED_DIR>.

However when I ran my second app server alone, this problem appears.

Do you have any idea that what should I do?

The BASE_PATH property (conf.suite.BASE_PATH=) should point to wherever you host the <Appian_home>/_admin which is the filesystem used by Appian:
forum.appian.com/.../Internal_Data.html

It might have gotten out of sync depending on how you install Appian. If the <Appian_home> directory was copied over from another location then this file(appian_home.properties) will be pointing to a path where Appian was originally installed.

Nothing in the App Server Log. I figured it out though... your BASE_PATH property in custom properties seems like it must match up to the AE_HOME property in the appian_home.properties file (both located in the ear/suite.ear/conf folder. Also, make sure the directory has the trailing "/" on it. Not sure how this got out of synch in the environment, but those changes seemed to do the trick.

A network trace can prove to be helpful.
Based on the symptoms it's worth to check if the URL matches the server and port property in the custom.properties file.

No Webserver, this is a development environment and is hit directly.

The X is just displayed by the browser not able to load the image. Do you use a webserver? Maybe something is wrong with the static content not copied to the right place ...

Agreed, that is how I validated the system.zip had the icons in it. It is very strange because it is not the standard broken image that is returned... it is actually a small x.

There are several other ZIP files. You can unzip a copy and have a look at the files. In one of them are the icon indicator files.

Just tried the suggested import of system.zip. No luck, it imported fine but did not correct the issue.
