If you manually add a column to the CDT via the editor (for example), it wouldn't reference any particular DB column though right? I assume this is the most likely (but of course not only) scenario when this question is asked.

Whilst that is a true design-time experience (i.e. adding a new column to a CDT will not magically change the database) if this CDT is used as a Datastore entity and the new CDT attribute references a database column that doesn't exist then the Datastore will not publish because Appian cannot successfully map that CDT to the database table.

Nothing will happen to the CDT until / unless you change it manually.
