Hi Aswini,

This issue might occur in the following cases.

1) Basic user has an AD account where the password was changed recent past and it will take some time to affect if you are using some third party login mechanisms like CA Siteminder logins or others.
2) It might be possible that user has saved the login credentials in the browser and after password change it has been re hitting again with old saved password. This may be accessed from other systems. This might be happened when the user is authenticated through some scripts from the other automated testing tools.

My suggestion here is like recreate the issue with user and check the apache and login csv log files. Otherwise for the R&D purpose you can make that user as admin and then try login and see if he face any issue and then change them back to basic again.

Please verify the settings in Admin console.Under Admin console - >Authentication -> Appian. You will able to see an option called Account Locking.

I've only seen this occurring when the user is hitting their maximum number of failed login attempts set in the Admin console. Have you checked the login-audit log files to determine if the user is hitting the number of failed login attempts threshold?
