Try the "Document Utilities" plugin I just shared in my prev reply. BTW, what problems you had with that plugin?

yes I search on plug in but I can not find any specfic plug in to convert XHTML to pdf so I try HTML plug in but it did'nt work out properly

How can we convert PDF from XHTML Because I try from Html plug in but it didn't work out any one answer me

hi,

With the same node (DOCX from XHTML), you could add in the "Template Document" input a DOCX file with the footer/header you want.

BTW, usually the conversion from DOCX to PDF modifies the alignment of the text and pictures, so, I would recommend to use a picture with all the components of your header or footer, instead of having text and multiple pictures.
