I think the issue is in your word document. I would suggest that you start with the template that comes with the plugin and slowly add fields as you go.

Well, then you somehow break the internal XML structure of the Word template. That's what you need to investigate.

It is a bit difficult to give any hints without knowing what you are doing. Try to explain it to a 5 year old kid
