If no error then it means the document can be deleted. That'd be the answer to your last question.

To disable the approval requirement take a look at the KC security forum.appian.com/.../Knowledge_Center_Security.html and its setting "All changes to the files contained in the KC require approval by the KC administrator"

Yes, from designer url. Now I am going to use Document Delete smart service in the process model after inserting the doc to folder. But my doubt is while going to delete with the help of document id, will it be deleted if the doc is not approved?

Hi Rodgraham. All folders having same security. And within same folder some docs are able to delete directly and some are not able to delete.

Have you checked the security settings on all of the folders? Is there a difference between folders where you can and can not delete documents?
