I am creating a POC Leave application, which requires employee, to perform the task of applying leave

Below is the employee interface to apply leave

When a employee submits a leave application along with leave details particular employee details should be displayed like emp id, name and department he works.

So please guide me in this.

The direct answer would be: Create a user and store the meta data of the user in the database.

But, when you share a bit more about your goals, we can give more and better advice.
