So in this example, I would need to add a new user into three groups? So I would be added into NAM Region group, USA Country group, and Dev Dept group? I need to create over 200 groups to cover every country/region?

You can store these details into one table namely user access and then create a view using this table and the main table which will join the rows with necessary matching values and will reduce the need to create group for each unique combination.

This is what groups are made for. Create normal groups for each Region, Country and Department and put users where they belong. The you can easily assign permissions etc.

Use user custom fields. Create Region, Country, and Department fields in Users -> Custom Fields, populate them for each user, then read these values using user() function to filter users and control access in your application.
