I suggest you to reach out to Appian Support in this case as configurations looks correct.

The model version is published, but the objects are not created.

The model should be published before Run Extraction button is enabled. So first publish then try!

Fix- You must click SAVE AND PUBLISH to make the model active. Simply open your model in AI Document Center, click the PUBLISH button, and the RUN EXTRACTION button will appear. Only published models can be tested.
