Task Reassignment questions

Hello, 
 We are implementing a solution where the users want task reassignments to include more details. 
 Our users would like reassignment to include a comment made by the person reassigning the task, and include the assignee before and after into the notification. This data should be made available to be saved as a CDT (insert) to a comment table. Can we access the reassignment process to change the notification? Also, can we setup a capturing this data near real time to save?

Hi charless88 as per my understanding, you need to implement custom reassignment feature in order fulfill your requirement. 
 
Where you need to have a picker field where the task will be assigned to the chosen user or group, where you can input your comment which will be shown to the reassigned user or group in readonly format
