I read

several times but it is still not clear for me whether I have to stop Appian Engines for backup or if I can hotbackup the environment.
In case of hotbackup, how to make sure the sync between kdb and RDBMS is ok?
Can someone help?...

You do not have to stop the Appian Engines during a normal backup, only before performing an upgrade. You do need to back up the application data files, KDBs, and RDBMS at the same time in order to ensure that they are synchronized. See:

Before taking a backup, you can run the checkpoint script and perform a backup procedure on the second-last kdb so you don't have to backup live kdbs.
