You can email Appian Technical Support or open up a support case under your organization using the 'Technical Support - Create a Case' action here on Appian Forum.
Then provide the output generated by following the steps here in the product documentation:

Appian Technical Support can then work with you on the license generation as well as ask for any other information they might need for this process.
