500 Error on SAML Logout

We've configured our install to authenticate a specific group using SAML. Everything works as expected, aside from getting 500 server error when we try to logout. We would like to solve this issue but it is not the main problem we are encountering. The main problem is that every few days the IdP metadata file becomes invalid resulting in all further access attempts getting a 500 server error due to not being able to reach the IdP (a result of the invalid metadata file). We haven't made any changes on the IdP or Appian side so not sure how or why the metadata file is randomly becoming invalid. When the issue occurs we have just been recreating the file, using the exact same configuration values, and uploading it again in the Admin console. After doing this it goes back to working for 2-3 days before starting the cycle again.

Anyone have any ideas on what might be causing this or what we can check? Any ideas on addition...
