High Swap Usage

We have a production server that has high swap usage, we perform housekeeping every weekend (Appian and JBOSS are stopped at this time). This has been getting worse and now (Tuesday) we only have 17% free swap.

The server is running on Suse Linux 11 in VMWare, has 2 vCPU, 16Gb RAM and 4Gb swap. What can be done to reduce swapping on this server?

Please advise if more information is required.
