I suggest to contact Appian support. But as far as I know they will not spin up these old environments ever again.

As Stefan said, they might not deploy these environments again.
Yes, they're being totally phased out in favor of the "beta edition" shared environment, which is nice other than not having access to admin, to the DB, or to creating or testing any Portals (sigh).
