Can anyone suggest how to add record action only as a page on site navigation bar? I believe it's only has a option to add record list. Please suggest.
Note: I am asking this because there is need to show popup on click of button or navigation menu.

So, as a record action is just a process with a start form, you could just add that process as a page.
While this might make sense, I do not understand your note. Can you help me with this?
I want to add page on site. This below screenshot a record list.
But I want to add only action which means when click on the menu or page it popups that process. That process added as a page won't work as expected because I want popup when click on the page.
Pages in sites cannot create a popup.
