I am quite certain that I have configured a Record Type Related Action correctly

I am quite certain that I have configured a Record Type Related Action correctly, but it is not displaying (a "No actions available" message displays). I have noticed the following error in the app server log when I select any of the Record Types on the Records tab. Could this be causing the Related Action to not display?
