Associating surveys with an existing record

 The above Survey Application shared on Forum states a feature as "Associate surveys with your existing records" -- has anyone implemented this? I'm just becoming familiar with how the app is designed, but would like some insight to how we could take the survey or results and associate them with existing records. 
 Should we just add hooks to the data in our existing records to gather the desired survey results? From a design approach, what is the best path? Thanks for suggestions.

I have not worked with that exact application but, the process of associating is quite easy. Simply add an attribute to the survey table that is recordId or similar and then you are able to easily query them based on that. 
 
Depending on your database normalization requirements, this may not be a complete answer. In the case of a requirement to be in 5NF or 6NF, you may need to use a reference table to complete this.
