The new Appian Community launches August 3, followed by Appian Academy on August 7. Since your environment is so old that Appian no longer posts the documentation specific to that version, I would just start with the appian/logs folder on the server, try tomcat-stdOut.log and go from there.
Otherwise, you could start investigating with the specific action if it is the same one each time, causing the issue? or different actions?
