We are currently running Appian in the cloud using MySQL. Some concerns have been raised regarding the scalability\\functionality of MySQL as our DB infrastructure. Does anyone have any experience running into issues with MySQL that they would like to share? What the issues were, at what point they were encountered, rough size of the application\\# of users, etc?...

From the start you should plan your database design well, eg. proper indexes, table design, etc.
