Have gone through the checklist. Was looking for some sample design document template.

By design do you mean look and feel guidelines?

By Design i mean the solution design for Appian BPM implementation in my project. It includes business processes, tasks, actions , CDT etc

While it doesn't actually include a template for the document, the wiki pages on the STAR Methodology describe a Functional Specification Document (FSD). Have a look here:
