I am using CreateCSVFromPVs Smart Service.
I want to insert an image into the generated CSV file.
How do I add it to the columns?...

I think this is not possible as CSV is a text only format.
If you use the CSV to import data into Excel you can try to create a HTML file like those generated by Appian when you click on the export button on the report header. When you put an HTML image tag into the cells which point to an URL where no login is needed (outside Appian) maybe these images are loaded by Excel.

I recently posted on creating files. Search for "xmlss". In short: Add a multiple text PV to your model, add some HTML to it and generate the file with the "text doc from template" smart service.
When you export a report using the "Export" button in the report header you can open this file in an text editor. Now you add an img tag into one of the td tags and open this in Excel. This works for me. Excel shows me the image in the cell.
