ALTER TABLE only allows columns to be added that can contain nulls, or have a DEFAULT definition specified, or the column being added is an identity or timestamp column, or alternatively if none of the previous conditions are satisfied the table must be empty to allow addition of this column

You can configure a column in the CDT as primary key in Appian, if the primary key is not specified then Appian tries to create a column as primary key.

Without any practical knowledge on this situation, I would suggest you to have non-null unique values in the database table column that you wish to mark as primary key and then try publishing your data-store ?

I would suggest making the changes in the DB manually before publishing.
Add the column
Populate the column
Add constraint to make the column a PK
Republish DS
