Implement the flatfile upload to DB and display values

Hi Team,
Could you please let me know if there is any sample example about Implementing the flatfile upload to database and display values from the database.
1. Upload the flatfile values to database
2. Interface form should display values from the database

Hi Priyanka,

Please find the updated package in which i have added datastore this time.
Inspect the same in your environment and you will see only one problem related to datastore publishment.

Note:
1) Please map the data source in datastore and then publish along with this you need to update DATA_SOURCE_NAME constant.
2) Make sure you have "Import CSV to Database" smart service which comes with "Excel Tools" smart services package.
For your reference i am attaching link to the mentioned smart service pakage in forum shared components
