We have created a process model by duplicating an existing process model , then added swim-lane and activity chaining between two nodes and saved and published the process model.
When we close it and reopen the process model, we are not able to see the changes (Swim-lane ,annotation and activity chaining), and connect lines too got distorted.
Has anyone faced this type of situation ?

Most likely the process model is referencing CDTs that have been deleted. Check out KB article 1001. Some related threads:
