Hi,
I tried to create an action against my process model. Even though the form was loading, the process model from the monitoring tab showed just a start node and end node. The actual process was not loading.

Try saving and publishing and see if that corrects the issue. If not, please make note of any message displayed on the bottom of the Process Modeler screen. It usually points the way to why the PM failed to publish.
If you saved, but you didn't publish, you would only see the last published version as an Action. If you never published it before, it might look like Start Node End Node.
