Issue with PDF Doc from Template Smart Service not generating correctly

I've created a process that utilizes the PDF Doc from Template Smart Service that was working correctly until this past weekend. There were no changes to the process model or to the template used to create our documents. I use to see a fully filled out pdf and now am seeing a blank page with empty boxes that display a value when clicked. Has anyone encountered this before?
