when the user clicks on action tab and click on any report and navigate immediat

when the user clicks on action tab and click on any report and navigate immediately to task tab, task tab created task to complete.Even after refreshing the task, it is still waiting to complete.

Only for particular one i am facing this issue and also when i check process, it is kicked off and not closed.please help me on this
