Process Model Issue

Actually we renamed process model from ###name1 to some other name, In application action we configured latest version process model.

When i click on my application action some times it trigger correct process model [Which i configured in application action] , But

Sometimes it triggering older version of process model , when we open the process instance in monitor mode it showing empty canvas but in process details
it showing older version process variables data.

Please help me for how to resolve the issue .

Since you renamed, is it possible something is referencing the pm by name? In a constant or otherwise. Did you verify action by repicking the right model?
I would also clear cache and maybe even bounce the dev server just to be sure everything is updated.

Yes i verified after repicking the action also,

recently we added latest appian hotfix , after adding hotfix these issue is came and before it's working fine.
In another env also we installed latest appian hot fix, there it's working fine.

When you renamed the Process Model, did you also save & publish the renamed model as well as any other dependent Process Models?

@Sally : Yes the process model save and published in Env-1[16.2] , later we moved application from Env-1 to Env-2[16.3 with latest hotfix] and
Env-3[16.3 with latest hotfix]. In Env-2 it's working fine but issue in Env-3.

Is the old process model that occasionally gets called still visible from /design or /designer?

After re-publish the process model it's working fine , But in another environment[Env-2] without re-publish the process model it's working fine
,same package we imported in Env-3.
The issue in Env-3 onlly why ? Is there Any specific reasons.
