(how to give "###" for checkbox") ...

Dhaval, what kind of report are you generating and what Appian functionality are you utilizing? By template are you referring to one of the generate document by template smart services? Typically a checkbox is Boolean value, and when saved into a PV it can be mapped to a template value.

In order to map such components in Word, one option is to generate the OOXML directly from Appian. For e.g. in your case, you will have ###PHONE###, ###EMAIL### ... etc., on the word document template. In Appian where you map these to the PV, you will have to generate the ooxml that shows the check box and map it.
Quick search came up with this link. Have a look to get an idea
www.schemacentral.com/.../e-w_checkBox-1.html
The value of checked or unchecked is directly managed in the ooxml

thanks for your comments.
can you please share me, In docx template file, do i have to put "###PHONE### and other checkbox fields only..? In document generator service, would generate the paramter mapping value as "w:checkBox" , is it correct ?
I have query here, how this "w:checkbox" will be appear as a checkbox file in generated document as it looks , it will just appear as a checkbox xml but not as a 'control' in generated doc.
thanks

When you map the code, it'll automatically be converted to the corresponding Word control. I have done this with table formatting but it should be the same with the check box control as well.
