It looks like the test PDF that you've attached does not contain any fields and I see the following message "If this message is not eventually replaced by the proper contents of the document, your PDF viewer may not be able to display this type of document. "
Try creating your PDF using another pdf creator. You can also use Microsoft Word if you're just trying to create PDF template and save as a PDF.

Patty, I created with Adobe lifecycle and i can clearly see in adobe reader and type the last name. Actually we have a financial pdf with many pages give by business which is created by adobe lifecycle which they use it everyday directly typing and i am trying to automate UI fields to fill in pdf and does not work with pdfdoc-to-template node. i cannot create that in word. i cannot attach that here in forum.

I am usign reader: Adobe Reader XI version 11.0.3

You should be able to use Adobe LiveCycle Designer to create forms compatible with the PDF Doc From Template node but it seems your form hasn't been created in a way that it can be saved. Every time I open the form I get the attached warning:

"You cannot save a completed copy of this form on your computer. If you would like a copy for your records, please fill it and print it"

That seems to be the root cause.

Make sure to remove this restriction and double check this form was stored in LiveCycle Designer as a Static PDF Form and remove any write/save and password protection.
