i have a scenario where i want to store document id in database, and in a result form i want to show in paging grid with appropriate link of that document so the a user can select the specific documents from the list of documents....

Once you upload document the get document Id and save in DB. To display in form , use query node or rule to fetch the same id and use document link function and name in second column by document function
