Please see a screenshot attached, I get this error often. Its an intermittent error. Closing and reopening task makes it working. This form does not have any complex java script on it. Any pointers to this will be really helpful.

If there is any JS on this, can you try removing it first and see if the form renders? Once done, if you still have the issue, you need to look into what the onLoad JS of yours does (if you have one).
