An Error Has Occurred com.appiancorp.kougar.driver.exceptions.Signal: index

Hi,

We are getting the below error when a user is accessing MyRecords page.

An Error Has Occurred
com.appiancorp.kougar.driver.exceptions.Signal: index

We verified the "appian-topology.xml" file as mentioned in the below KB in both app & engine server, but it didn't help.

Any idea on why this happening.
