We have designed a Email Template which is HTML. When this template coming up in email message, it is loosing some controls like Radio Buttons, Check Box etc. This problem is in Outlook and the same works fine in GMAIL.
Any suggestions to resolve this ?

which says that Outlook simply ignores a lot of HTML tags.

I don't think there is a way to get this to work in a reliable way. Email is technically a quite complex topic and how mail clients show them is even more complex.

Use case - We don't want end users (One of types) to login in Appian but their inputs want to capture in Appian so we did email integration and looking 2 or 3 inputs. This the format which we thought but we know there could be simple text format if this doesn't work. Only requirement is to what is best way to collect inputs.

You could create a input form in an external system like SharePoint or a web plattform that collects the data and submits it to Appian via Webservice.
