Functional checkbox field in Dynamic Word Template

Hello Everyone,

I have a requirement to add a functional checkbox in the word template. I have added the checkbox through 'MaccroButton' and 'checkboxformfield' but did not get any way to implement the 'tag' to pass the data through the xmlDataModel. Is there any way to achieve this ? Can anyone suggest a way to achieve this?
