I have a PM when launch would pop up a window for CSV file upload. However instead of getting upload file feature, it's asking me question and I am unable to upload a CSV file? What is the issue here:

That's because you have a start form. Just open the model then go to File - Properties - Delete Start Form

Make sure your start event is chained to the task where you enabled the Upload File component so that this task is the first you see.

You may have defined 2 PVs as parameters. Appian will create a start form by default in this case (with default labels and type based on PV data type). If you want to use SAIL file upload component (a!fileUploadField), you can replace the default start form with a SAIL form

Hello Surendra, Adding to the above, In case you have a form chained from the start event to user input task, but you have two PV defined which are set as parametric then Appian will create a start form by default, in that case, you can delete the start form and publish it.
