I am trying to fill FDA 1571 form using Appian and using the Configure PDF doc from template in Document Generation category. It seems that Appian is unable to parse this Form.
Please help.

This PDF is locked and cannot be edited as a form.
It may be that Appian cannot reach the field level meta data.
If possible, obtain an unlocked copy and try again.
