If the CDT was updated and it has been added to the application, you can use the “Impact Analysis” tool located in System > Data Types (it appears after checking the CDT that is being erased) to know which components are affected in order to update them once the new version is imported.

I am referring to running process, will it get updated with modified cdt or variables?

Running process instances will not be updated if you are to make changes. These instances will continue to run on the version of the cdt/variable which was in place at the time it was started.
If you need to update running processes, then you can consider creating a custom smart service plugin using the process upgrade API's:
