How to start a PM in debugging mode?
I went through File->Start PM in Debug Mode->Right clicked on my first highlighted userinput task in debig mode-> clicked open form
Instead of opening a form its gives a screen and a link to open form in a tempo (image of this screen is in attachment) and when I click the tempo link it goes to news and not to the user input form.

Hello, the node in question looks to be a user input task that is using a SAIL form (that's why it must be viewed in tempo). Please check who that task is assigned to and then log in as that user and click on your tasks to see the form.

@saileshd I would like to suggest to navigate to the Tasks tab to act on the corresponding task. Just in case, if you think that you are not the task assignee, you could make yourself a task assignee from the user input task node in the process or by debugging the process details.

As that user input task refers to an Action and not a task, I went to the Actions tab (I cross checked If I have anything thr, but there was no new task assigned to me). The task is already assigned to me. Even when I initiated that particular Action through Actions TAB, the process model did not show any response based on my inputs

This form was created by Appian Professional Services. Is there a way for me to find out which type it is?

Open the process model (not the instance) in the process modeler, double click the user input task and click on the Forms tab. That should give you the information on whether or not the form is SAIL or forms designer.
Can you also check the Assignment tab and let us know who this process is assigned to? As someone mentioned earlier, you can navigate to the process instance and reassign the task to yourself. You would then be able to see this task in your own task list in Tempo

The User Input task appears to be a quick task (Tilda icon in the bottom right of the activity). Quick tasks would appear as a related action on a process backed record. If you open the UIT and uncheck the "qucick task" checkbox on the general tab, this should show up in your task queue.
