Based on some research this seems to be a transaction timeout and not an install issue.
Try increasing the timeout value as suggested here in the ds file:
ttlnews.blogspot.com/.../jboss-exception-transaction-is-not.html

Any idea why we would be seeing this in one environment and not the other? Same install, same app, same config, and only 2-3 more users.

Maybe it's related to the amount of data the queries are fetching from the DB ?

Nope - in fact less data in this environment

Is there any warning or any other error before this starts to happen ? Maybe it's an effect of something else ? Also, it would be worthwhile to check if there is anything logged on the Oracle side.

Nothing else logged or indicated on either side..
