We experienced some odd behavior in our production environment today. About 30% of the users had trouble 'accepting' tasks. A blank page would display with "accept task", Logging out of the application seemed to resolve the issue but since this happened before I wonder if there is an underlying error.
I found this 404 error on the app server log:
2013-08-20 20:36:02,691 [http-0.0.0.0-8080-9] ERROR com.appiancorp.ap2.PortalResponse - Error: 404
Message: /suite/portal/img/skins/default/logo.gif

file for the Document could not be found on the server. (id=298, path=d:/appian/_admin/accdocs1\\0\\-1.)
Any advise would be great. Thanks. ...

can you ensure all your engines are running properly (run checkengine.sh/bat under /server/_scripts/diagnostics directory.
Also worth finding out if your servers resources are not maxed out OR any other services (such as anti-virus) is not locking the kdb's etc.,

Bruce, did the reported issues occurr during a specific time frame? Check the gw logs for the engines to see if anything occurred during that period. This might provide a clue as to why the issues were reported.

Sathya- engines ok and server resources not maxed out. The anti virus will be checked in real time the next time the event occurs.
Michael- we reviewed the gw logs (16 of them) around the time the events occurred and found no anomalies or errors. Our working knowledge pertaining to these logs is limited. thanks bruce

Do you have a web server in front of your app server? Any known outages at the web server layer, even momentary?

Venky- nice to hear from you. I believe we use apache web server, but I will confirm with our sys admin.

Hi Bruce,
If you are indeed using Apache HTTP server, please navigate to <Apache_Home>/logs. Is there any corresponding error message from the
---> access.log
---> error.log
---> and if you have SSL configured, ssl-error.log?
