I'm in the middle of training exercise 8, and have created this process model on the Community site. However, when I Start the process for debugging, the Start form does not appear, but the process just goes on to write nothing to the database. Any ideas please?

I am suspecting you may not publish your process. Publish your process and try debugging, it may resolve your issue. Before publishing make sure validate your process(By Right click on inside process model -> Validate) to check there any errors or to see any recommendations.
You cannot start a process in debug a process unless it is published.

That is right, without polishing a process you cannot debug that process. But if we already have a published version and we make modifications to existing process and save that process(didn't publish), now if we try to debug the process(with start form) it won't use latest saved process model (Since latest modifications are not published), instead it will use previous version. That is what I try to mentioned.
