Check Box is not visible in Form for PDF created by Generated PDF plugin.

We have created PDF with Check box using Generate PDF plugin. Check box is visible in PDF but when we view the same PDF thru Appian SAIL Interface. All Other Information are visible except the check box. Please advise and any help on this will be appreciated. Please find the attached Document with all Details.

@jegadeeswarand, please check both are same doc ids.. it is not happened when you fetch same doc ids
