Seems like someone already submitted the task by filling in the content, I think submitted tasks cannot be retrieved

Which Appian version are you working on?
1. If you are working on SAIL code, issue would be some thing wrong with your code.
2. If you are working on apps portal/portal dashboards then, this error pop up when the button is clicked twice.

Note that Submit button should not be double clicked.
To prevent double submit you can hide the button as part of the submit event of the form. This approach will hide the button once it is clicked avoiding multiple submits. In order to do it you can use: window.FormAPI.hide("button1"); where button1 has to be replaced with the ID of your submit button.

Please do the following:
0. Verify the issue is not that you're clicking twice in the submit button just like Ramesh is suggesting. If the task is indeed submitted and removed from your tasks list then probably this error occurs because of multiple clicks on the submit button.
1. If this environment has a dual gateways configuration please ask your Administrator to verify that the checkengine output shows 2/2 Okay for all the engines.
2. To make sure the reports are showing the right information and that there's no issue with the reporting engine (Analytics) ask your Administrator to re-create Analytics (she should know how to do this already) to see if the issue is resolved.
