How to add Checkboxes & Signatures in Dynamic Docx Generator

HI All,

I have a requirement, where we need to generate the Dynamic Word Template. we are using "DOCX from Dynamic Template." How to add signature & check boxes in the word template dynamically

Signatures: Lets say I have an Employee editable grid, whenever we add an employee, signature will be provided by employee, how we can show those signatures in the Word Template. 

In the Employee form we have a checkbox field how we can represent that in Word Template.
