Insert Dynamic list of images into Dynamic DOCX word template

I have a requirement to dynamically generate a word document from a template and will need at a certain point to insert images in different places.
i have a list of assessments and for each assessment will need to generate a map picture of the location of the assessment (the number of assessments can vary and is not static)

i was able to bring the list of assessments information but have no idea how to bring for each assessment the right map image generated ?

i have attached the following screenshots for more clarity 

When i try [#list logo as logo] ... [/#list] it insert all the images in one column like the following

Please any suggestion how to solve this issue
