An uploaded document can only be accessed AFTER submitting the form.

Hello Friend, 

Just like documents, Appian will not reflect the implementation of the uploaded image UNTIL the user has submitted the form where the upload has taken place. Once this has been done Appian will generate an ID to this image and save it to the KC / folder you designated; now, you can view the image attributed to the specified user.
