Display Checkbox Values in PDF By Using PDF Doc From Template smart Service

"I am having trouble displaying checkbox values in a PDF generated by Appian. I am using a PDF Doc Template, and the checkbox values are not displaying as expected. Here’s a screenshot of the issue. Can anyone help me figure out what I might be missing?"
