Interface is not getting displayed for process initiator

I'm facing two issues:

1. Warning is coming in process instance - Some referenced items could not be found. Please check the model and correct any problems.
2. Action Complete banner flashes out..

Hi, looks like interface is not able to detect generated document. Can you check if you have valid doc id in process and if yes then check if interface is getting that document correctly.

    Hi, yes i'm getting the document on process variable which being passed to interface.

    Note: when i goto process instance and view form by clicking user input it working fine.

    can you check your code in designer and pass the doc id and check?

    Did you enable chaining from the first interface or start node to the second interface?

      Check the assignment settings and have the activity chaining.
